Screening for Down’s Syndrome

Hillsborough Private Clinic offer an option for mothers interested in screening their babies for Down’s Syndrome through a blood test called a Harmony Test. Unlike other screening options, this does not put your pregnancy at risk, as it is completely non-invasive and has no effect on your child. This test is available from 10 weeks and is > 99% accurate. This test will also predict the sex of your baby.

Menstrual Dysfunction Symptoms

Any of the following symptoms can be known as menstrual dysfunction:

-Abnormal bleeding / prolonged menstrual bleeding

-Fibroids, non-cancerous uterine tumours

-Premenstrual syndrome (PMS) which can cause both physical and emotional discomfort prior to periods.

Likely Methods of Treatment

Menstrual dysfunction treatment is unique to the individual and the nature of the dysfunction. For example, abbnormal or prolonged bleeding can typically be helped by oral contraceptives only when prescribed by the GP or a Gynaecologist. Meanwhile, light or absent periods can be helped by hormone therapy and lifestyle changes, which may be advised too.

Occupational Therapy helps people to do the everyday things that they want to do or need to do when faced with a physical illness, injury or disability.

Located in Orthoderm Private Clinic, Hillsborough, our Occupational Therapy team consists of qualified occupational therapists with many years specialising in hand therapy.

What to Expect During Your Occupational Therapy Appointment

We provide specialist hand therapy for people who are experiencing problems with their hand, wrist or elbow. We work closely with our clients to complete detailed assessments and develop individual treatment plans to encourage a speedy recovery from injury, prevent recurrence of symptoms and even slow the progression of chronic conditions affecting the hand and upper limb.

 

Your Occupational Therapist will conduct a detailed evaluation of the structures of your hand and upper limb, including the assessment of muscles, tendons, ligaments and nerves. 

Treatment programmes are tailored to each individual and vary depending on your condition or recent surgery. 

We use a holistic approach, considering the impact of the presenting condition on your ability to perform daily tasks at home, in work or school or during social, leisure activities and sport. We provide specialist treatments to improve performance in these areas.

Examples of hand specific treatments you may receive include wound care, scar management, therapeutic activities, exercises to improve quality and range of movement, strengthening and conditioning programs custom-made and prefabricated splints, pain management modalities and dry needling/ medical acupuncture.

Elaine is a qualified Occupational Therapist from University of Ulster in 1994. She started her career in the Ulster Hospital before migrating to New York where she spent 11 years in some of the most prestigious hospitals.

Cubital Tunnel Syndrome

Cubital Tunnel Syndrome

Cubital Tunnel Syndrome is compression or irritation of the ulnar nerve in a tunnel on the inside of the elbow (where your funny bone is).  The ulnar nerve provides sensation to the little finger and part of the ring finger, and power to the small muscles within the hand. Most cases arise without obvious cause, but the tunnel can be narrowed by arthritis of the elbow joint or by an old injury. 

When Cubital Tunnel release surgery may be appropriate

You may require surgery, known as cubital tunnel release, if you experience pins and needles or numbness that comes and goes in your little and ring fingers, or pain on the inside of your elbow.  Investigations may include x-rays of the elbow and nerve conduction studies.

How is Cubital Tunnel release surgery performed?

Surgery required to decompress the Cubital nerve can be performed under a regional block, where the arm is anaesthetised, and you will be wide awake.

Cubital Tunnel release surgery is a day case procedure. The surgery will involve an incision around the inside of your elbow. The roof of the tunnel will be exposed, and the nerve released (decompressed).

Your surgeon will advise on the technique most appropriate to your problem.

The success of Cubital Tunnel release surgery largely depends on the extent of nerve compression and the duration of symptoms before surgery. Recovery times can vary, and your surgeon, along with a physiotherapist, will create a tailored treatment plan to optimise your recovery.

Dr Edgar Boggs, Consultant Gynaecologist and Obstetrician

Dr Edgar Boggs has over 20 years’ experience in Gynaecology and Obstetrics. Specialising in pelvic floor disorders and minimally invasive surgery; i.e. hysteroscopy. Dr Boggs is a Consultant Gynaecologist in Craigavon Area Hospital and Daisy Hill Hospital, Newry. Dr Boggs graduated from Queens university in Belfast in 1999. He spent 2 years in Toronto completing his fellowship in advanced gynaecology in pelvic floor care. Dr Beverly Adams, Consultant Gynaecologist and Obstetrician

Dr Beverley Adams has almost 25 years’ experience in Gynaecology and Obstetrics. She graduated from Queen’s University of Belfast in 1997 and was awarded Fellowship of the Royal College of Obstetricians and Gynaecologists (RCOG) in 2019. She was awarded a Masters degree in Medical Law in 2010.

Dr Adams has been a Consultant in Craigavon Hospital since 2009 with experience in general gynaecology and an interest in postmenopausal health and paediatric/adolescent gynaecology. She also offers specialist medicolegal opinion for Obstetrics and Gynaecology legal cases.

She works within a team of doctors in Craigavon who offer care to women in pregnancy when structural fetal problems have been identified and attained the postgraduate Diploma in Obstetric Ultrasound in 2002, awarded by the Royal Colleges of Obstetrics and Gynaecology and Radiology.

Dr Adams is one of the team of Consultants in Craigavon who have been recognised by the RCOG for excellence in postgraduate training, most recently in 2022.

Dr Knox qualified from Queens University Belfast in 2004 and completed his training in Northern Ireland in 2013. Following this he undertook a fertility fellowship in The Merrion Fertility Clinic Dublin. Dr Knox has worked as a consultant in Craigavon Area Hospital for 10 years and is the current Clinical Director. Dr John Manderson is a Consultant Gynaecologist and Obstetrician at the Ulster Hospital, Dundonald and Lagan Valley Hospital, Lisburn. Dr Manderson is a member of the Ulster hospital Obstetric’s and Gynaecology on call team. Dr Manderson specialist interests include high risk pregnancy management and foetal scanning, completing his Doctorate in high risk pregnancy and further sub-specialising in Fetal Medicine in Belfast and Toronto being one of only a handful of consultants in this field in Northern Ireland. Mrs Sarah Morton, Dietitian

At the helm of Food Script Nutrition is a Sarah Morton, a highly specialist Dietitian with 13 years' experience in both the NHS and private practice.  Passionate about health but also about our relationship with food, Sarah will bring her many years of behaviour change techniques, significant clinical knowledge but most importantly, her compassion to provide an individualised bespoke plan based on a client's specific nutritional needs.

Sarah is a specialist in disordered eating including diagnosed eating disorders (Anorexia Nervosa and Bulimia Nervosa) in adolescents and adults.  Sarah also has significant experience in working with binge eating disorder and general weight management concerns, as well as extensive experience in functional bowel disorders (IBS) and low FODMAP diets.

Dr Lee qualified with a Medical Degree in 2012 from Queen’s University Belfast. She attained MRCGP in 2019 and has worked as a GP Partner. She achieved the Diploma of the Royal College of Obstetrics and Gynaecology and took up a post as a GP with Special Interest in Obstetrics & Gynaecology in 2019 in the Southern Health Trust, working in Gynaecology and Antenatal Clinics. Coil Insertion/Removal

Coil Insertion/Removal

Our coil clinic specialises in both the insertion and removal of intra uterine devices (IUDs) and intra uterine systems (IUSs). The coil is a long-standing reversible contraceptive option that is inserted into the uterus.

There are two types of coils:

  1. Copper coil - IUD
  2. Hormone containing coil (Mirena) - IUS

Both types of coils are one type of long-standing reversible contraception that is inserted into the uterus.

Our clinicians will discuss the entire process with you before the coil insertion procedure begins to ensure that you are as familiar, comfortable and relaxed as possible. For those wishing to have their IUD removed, we also offer coil removal procedures.

Types of Coils

The copper coil, also known as an intra uterine device (IUD), works by stopping the sperm from fertilising the egg and therefore preventing implantation. This lasts between 5 and 10 years. 

The hormone containing coil (Mirena) is also known as the intra uterine system (IUS). It works by releasing a hormone that thickens the cervical mucus and thins the lining of the uterus. Although 99% effective, Mirena coil is not suitable for everyone, but this will be decided by your Consultant. It is safe to use whilst breastfeeding. After removal of the coil, fertility returns to what it was before insertion. It can make periods lighter, shorter, and less painful.

Coil insertion procedure

A pelvic examination will be performed prior to the coil insertion by your Consultant Gynaecologist. The doctor will gently put a speculum into the vagina, similar to a cervical screening. The IUD/IUS will be inserted into your uterus through the cervix. Always let the doctor or nurse know if you feel any discomfort and the procedure can be stopped at any time.

Having either type of coil fitted should take between 5 and 10 minutes, but we advise patients to allow 30 minutes for the appointment. Some people find having an IUD/IUS inserted painful, so it is recommended that you take over the counter painkillers like paracetamol or Ibuprofen prior to coming for your insertion appointment.

Aftercare

After the coil is fitted, you might experience cramps or period-like pain so take over the counter pain relief like paracetamol or Ibuprofen when required. There may be some light bleeding or spotting at this stage.

An IUS/IUD has strings attached which hang down a little way from your uterus into the top of the vagina which will need to be checked regularly to ensure that the device is still in position. 

You will be taught how and when to do this.

If you cannot feel the threads, it might mean that the device is no longer in the correct position. You must contact your doctor and use another form of contraception until you discuss this with your Consultant.

A review appointment can be arranged after the coil is fitted if required.

Coil removal procedure

During the coil removal procedure, the coil will be removed by the doctor in a process that will be very similar to the coil being fitted. Again, pain relief is advised before coming for the appointment.

Once the coil is removed, fertility is restored immediately so if pregnancy is not wanted, please use another form of contraception. There can be a little bleeding after getting the coil removed, but this should subside within a few days.

Types of Contraception

Mini Pill

The mini pill is a progesterone-only pill which raises the level of progesterone in your body and therefore stops ovulation. The mini pill contraception option comes in packs of 28 and is to be taken daily. 

Contraceptive implant (The Bar) 

The contraceptive implant (also known as the bar) is a small tube that is inserted into the inner part of the upper arm. This tube releases a low level of progesterone into the body, which helps to stop ovulation.

Contraceptive Injection

The contraceptive implant, or Depo Provera, is a large dose of progesterone given every three months. This injection stops ovulation.

The Coil

The coil is a copper coil, or intra uterine device (IUD), is a hormone-free option which is inserted into the uterus and prevents sperm from fertilising the egg. Alternatively, another option is the Mirena coil, or intra uterine system (IUS), which is placed in the uterus and releases hormones to thicken the cervical mucus and thins the lining of the uterus.

Condoms

Condoms help to prevent both pregnancy and sexually transmitted infections (STIs).

Tubal Ligation or Vasectomy

Both tubal ligation (surgical clipping of the fallopian tubes) and vasectomy are very final methods of contraception. These should only be contemplated once you are certain that your family is complete.

Polyps & Fibroids Treatment

Polyps & Fibroids Treatment

Our clinic offers diagnosis and treatments for patients with uterine polyps and/or fibroids, helping you better understand the condition and treatment options available.

Very often uterine fibroids and polyps do not have any symptoms and are common growths of the uterus. In those who do have symptoms it can be influenced by the location, size, and number of polyps and fibroids. 

Some polyps and fibroids may resolve on their own or maybe not require any form of treatment if they are small and asymptomatic. They will be monitored as per the gynaecologist. This is called watchful waiting.

Uterine polyps

Appearance of uterine polyps may be linked to changes in hormonal levels. Each month the oestrogen levels rise, and fall causing the lining of the uterus to thicken and then shed during menstruation. Polyps form when too much of the lining grows. When they grow larger, uterine polyps treatment options may become necessary.

A woman might be at higher risk of polyps due to:

-Obesity

-High blood pressure 

-If they have been on the breast cancer drug Tamoxifen.

Common Symptoms of Uterine Polyps are:​

-Heavy, painful, or irregular periods

-Vaginal bleeding after menopause 

-Frequency in urination 

-Constipation

-Pain in the stomach or lower back during intercourse 

-Bleeding between periods 

-Tiredness due to anaemia 

-Infertility problems

Diagnosis and Treatment of Uterine Polyps

Uterine polyps can be diagnosed and examined during a Transvaginal scan done in the outpatient setting at the patients first appointment with the Gynaecologist (this can be done if you are having your period on the day of the scan).

At this stage the consultant will decide whether a hysteroscopy is appropriate or not. Hysteroscopy will not be done on the same day as your initial outpatient appointment. There is a likelihood that the polyp will be removed during hysteroscopy procedure.

If there is a requirement for an endometrial biopsy the tissue will be sent to the labs for analysis.

Polyps Aftercare

You will come back to the clinic for a follow up appointment if deemed appropriate by the Consultant. At this appointment any specimen results will be discussed along with other matters related to your care. You will be discharged from the care of the Consultant if appropriate.

Fibroids

You will come back to the clinic for a follow up appointment if deemed appropriate by the Consultant. At this appointment any specimen results will be discussed along with other matters related to your care. You will be discharged from the care of the Consultant if appropriate.

Gynaecology Services

There are many reasons why you may need to see a gynaecologist including your sexual health, reproductive health and general wellbeing. At our clinic we offer a range of gynaecology services to ensure quick diagnosis and effective treatment.

Ultrasounds/Abdominal and Vaginal

This is done routinely at an outpatient gynaecology appointment. A probe is used to scan abdominally to check the uterus, ovaries, and bladder. If polyps, fibroids or cysts are discovered, your Consultant will give you further advise on these. Pregnancies can also be detected at this stage. 

If a more detailed scan is needed a probe is gently placed inside the vagina by the gynaecologist. This is very routine and not a painful procedure.

Hysteroscopy

Hysteroscopy is used to diagnose and treat various symptoms such as bleeding, fibroids, and polyps. This is a short procedure performed at our clinic by one of our expert Gynaecologist and takes no more than 15 minutes. A camera is placed inside the vagina and is used to look inside the uterus.

Your Consultant will be able to give you the result of the examination immediately unless they have to take samples of tissue to send to for analysis. These results can take up to a month to be returned to you. A follow up review appointment will be arranged where you can discuss your results with your doctor and any concern or questions you may have. 

Hysteroscopy is performed in most cases without local anaesthetic or general anaesthetic, but we can provide local anaesthetic and sedation in Orthoderm Private Clinic if necessary.

Endometrial Biopsies

This procedure can be performed as an outpatient and is similar to a cervical screening test. It involves taking a small tissue sample of your uterus lining and is used to diagnose the cause of heavy menstrual bleeding, abnormal cells and certain forms of cancer. It is a quick and easy procedure that lasts no longer than 15 minutes. 

You may experience mild discomfort and cramping while the sample is taken and a short while after the procedure. This sample is then sent off for analysis which can take up to a few weeks. Once we have your results a review appointment will be arranged, where your Consultant will discuss your results with you and the next steps in your treatment if required.

Alternative Treatments

All individual patients at our clinic are different and may require different forms of Gynaecology Services and treatment that is individually tailored to their needs. Our specialist Consultants Gynaecologists are best to advise you on appropriate treatments and care.

Alternative treatments can include diet and lifestyle changes, and maintaining a healthy weight. Oral contraceptives or other non-hormonal medications can be prescribed to keep your periods in check. While vitamins and iron supplements may also be advised to help alleviate your symptoms.

Here at Orthoderm we provide urogynaecology appointments for a range of different urinary, bladder and pelvic issues, offering expert care and treatment options to our patients. 

Urogynaecology is a subspecialty of gynaecology which concerns the pelvic floor. It covers problems of the lower female urinary tract and genital tract. 

In many cases prolapse of the pelvic floor organs, including the womb and bowel, can affect your bladder. This can lead to an overactive bladder and urinary and faecal incontinence. 

In a lot of cases, symptoms can be relieved with specific medication, physiotherapy, pessaries and, in some cases, surgery.

 

The Cause of Urogynaecology Conditions

Many women who attend a private urogynaecology appointment will have experienced some form of bladder control issues. This is usually related to every day activity like walking, running, other exercises or even sneezing and coughing. 

Bladder over activity, also known as frequency or urgency, which can prove to be very debilitating on a day-to-day basis, causing anxiety, stress and sometimes depression as some people feel unable to leave their homes.

Other women might suffer from related problems such as genital tract prolapse, vaginal dysfunction and recurrent urinary tract infections.

There are two main types of incontinence.

-Stress incontinence - These are leaks that happen when we laugh, cough, sneeze, or exercise.

-Urge incontinence - These are leaks that happen when we feel a strong urge to empty our bladder, but we don’t make it to the toilet on time.

Physiotherapy and Pelvic Floor Exercises

Fortunately, most women can improve or cure their urogynaecology conditions with physiotherapy and pelvic floor exercises. 

The pelvic floor muscles are a group of muscles that sit at the bottom of the pelvis. They run from the pubic joint at the front and attach into the coccyx bone at the back. 

There are many muscles in the pelvic floor and are found in different layers. These muscles all work together to keep us continent and support our pelvic organs.

There are 3 openings in the pelvic floor: 

 -Urethra at the front – this is the opening for the bladder where urine passes through

 -The vagina in the middle

 -The back passage at the back

These muscles also support the pelvic organs, the bladder, the womb and the bowel.

The Causes of a Weaken Pelvic Floor

Many factors can lead to a weaken our pelvic floor, including:

 -Pregnancy and childbirth

 -Constipation/ straining when trying to go to the toilet

 -Menopause

 -Surgery

 -Heavy lifting

 -A weak pelvic floor

If these muscles become weak, it can lead to urinary incontinence and can affect up to 1 in 3 women in their life.

 -For recurrent urinary tract infections your Consultant may suggest medication, voiding techniques or intermittent self-catheterisation if you have a residual bladder.

On Wednesday, 19th June, the Orthoderm Clinic, Room One Physiotherapy, and Affidea Belfast will host a CPD event at the Culloden Hotel and Spa in Holywood, Belfast. The event will welcome GPs, FCPs, and AHPs.

The evening will commence with light refreshments at 18:00, followed by presentations starting at 18:30. The event will feature two distinguished speakers:

  • Mr. Kevin Donnelly: He will present "An Update on Shoulder Arthroplasty and Shoulder Osteoarthritis Management," covering joint replacement options for the shoulder, post-operative rehabilitation, and the management of end-stage arthritis.
  • Dr. Noel Napier: His talk, "An Update on Imaging of the Pelvis and Hip," will provide a comprehensive review of pelvic and hip imaging, discussing conditions from infancy to the elderly.

Procedure - Fasciectomy

Fasciectomy Surgery for Dupuytren’s disease

Dupuytren’s disease, also referred to as Dupuytren’s contracture, is a common condition that usually arises in middle age.  Firm nodules appear in the ligaments just beneath the skin on the palm of the hand. Dupuytren’s disease often begins with nodules in the palm, frequently in line with the ring finger.

The nodules extend to form chords that pull the finger towards the palm and prevent it straightening fully.  One or more fingers may become fixed in a bent position. The cause is unknown, but it is more common in Northern Europe and it often runs in families. This condition can be treated through fasciectomy surgery.

How is Fasciectomy surgery performed?

Fasciectomy can be performed under local anaesthetic (numbing the area of the affected fingers and palm), or regional anaesthetic (when the whole arm is numb) and you will be wide awake. Surgery is performed by removing the abnormally thick and fibrous tissue in the palm/fingers through a zig zag incision on the affected hand. Surgery usually provides relief from contractures and allows you to move your fingers again.

When Fasciectomy surgery may be appropriate.

You may require Fasciectomy surgery if one or more fingers become fixed in a bent position. Contracture of the fingers is usually slow, occurring over months and years rather than weeks. Surgery can usually make bent fingers straighter, though, not always fully straight. Our Occupational Therapist and Physiotherapy team will work with our surgeons to design the best treatment pathway for you.

What to expect after Fasciectomy surgery

After your surgery you will have a bulky padded bandage or a plaster backslab, which you must keep dry. Your stitches will most likely be dissolvable.  It is important to keep your hand raised as much as possible to reduce swelling. Keep the bulky bandage on until you see the Hand Therapist approximately one week post operation to aid recovery of the hand. 

You can start driving as soon as you feel confident enough to control your car safely and you are no longer wearing the splint during daytime – approximately 2-3 weeks after your surgery. Returning to work depends on your job and your excerpt consultant or therapist will be able to advise you.

Cubital Tunnel Release

Cubital Tunnel syndrome is where the ulnar nerve becomes compressed on the inner side of the elbow. This can cause numbness, pins and needles and tingling in the ring and little fingers. It could also cause the many muscles of the had to waste away and become weak which can cause loss of function. The operation to release the nerve is done with a small incision where the tight tissue on top of the nerve is released. This gives the nerve more space and allows it to function more normally

At the helm of Food Script Nutrition is a Sarah Morton, a highly specialist Dietitian with 13 years' experience in both the NHS and private practice.  Passionate about health but also about our relationship with food, Sarah will bring her many years of behaviour change techniques, significant clinical knowledge but most importantly her compassion to provide an individualised bespoke plan based on a client's specific nutritional needs.

Specialising in disordered eating including diagnosed eating disorders (Anorexia Nervosa and Bulimia Nervosa) in adolescents and adults.  Sarah also has significant experience in working with binge eating disorder and general weight management concerns.  Sarah also has extensive experience in functional bowel disorders (IBS) and low FODMAP diets.

In particular Sarah believes the issues around life stage nutrition and hormonal nutritional concerns require a wealth of sensitivity as well as individualisation to ensure that a client's advice is as individual as their journey getting there.

What Is The Cross Border Healthcare Directive?

The Cross Border Healthcare Directive (CBD) was a piece of EU legislation that entitled patients in Ireland to avail of faster treatment or surgery across the border in Northern Ireland or in any other EU state. This is now operating under new legislation as the Northern Ireland Planned Healthcare Scheme. As a result, access to Cross Border Healthcare in Northern Ireland for patients from the Republic of Ireland will continue as normal under the NEW NI Planned Healthcare Scheme throughout 2021.

Patients looking to avail of faster treatment or surgery across the border in NI will be able to follow the same pathway as under the previous healthcare plan and will be granted reimbursement of their treatment costs through the HSE.

Steps to Access Treatment Under Cross Border Healthcare:

Step 1: Phone 028 9268 0940 (ROI 0044 28 9268 0940) today to speak to our team in Orthoderm or email appointments@orthoderm.co.uk. You may already have your referral letter from your GP or Consultant for the procedure you require. If not, the team can talk you through what you require on your referral letter. Our team will give you all the details about pricing and any access that may not be reimbursed. We will ensure everything is clear before you decide to avail of our services.

Step 2: You will be assigned a consultant for your specific procedure and provided with a date, time and location of your appointment. This will also be confirmed in a letter by email/post. You should also keep copy of any recent (6 months) x-rays/scans.

Step 3: You will attend your consultation appointment in Orthoderm where all paperwork will be arranged for submission to the HSE.

Step 4: A surgery date will be agreed along with payment options.

Step 5: You will attend for your procedure and will be discharged with a pro forma invoice to be posted to the HSE for reimbursement (generally within 10 - 12 weeks), post-op discharge plan, and any relevant information required should patient experience post-op complications. It is advisable to purchase something small in a local shop and keep the receipt, this can be useful as proof of your attendance in NI.

Prof Dr Melania Anna Duca is a highly qualified Consultant Psychotherapist with several years of experience both as a clinician and in behavioural science research, now practising at the Orthoderm Private Medical Clinic, Hillsborough. Among the modalities offered at Dr Duca’s practice are a few innovative tools for mental health treatment such as virtual reality exposure therapy within the context of a psychoanalytic approach to mind investigation, which is found to be very helpful in cases of psychological trauma, generalised anxiety disorder in adults, clinical depression, stress and anger management, and sport performance. Dr Duca specialises particularly in high functioning autism and long-term prognosis such as Asperger syndrome and psychological support to late diagnosed “gifted” children.

The Orthoderm Clinic is pleased to offer a Private GP Service.

Do you need to see a GP?

Everyone can access our private GP service. We offer a full range of GP services, with additional access to x-ray, MRI, and physiotherapy services, and direct access to specialist consultant referrals if required.

Do you need to be treated urgently for a minor injury?

The Orthoderm GP service can treat minor injuries such as minor burns and wounds, sprains and strains, minor head injuries, joint, muscle and back pain.

Do you need to be treated urgently for a minor illness?

The Orthoderm GP service can treat fever and rashes, ear, nose and throat conditions, chest infections and stomach problems.

Are you worried about a health problem?

The Orthoderm GP service can offer health screening with a full range of blood tests, ECG, blood pressure checks, x-ray and MRI investigations.

Are you worried about a sexual health issue?

The Orthoderm GP service offers sexual health and GUM advice and treatment. We offer family planning and female and male health services.

Do you need a pre-employment medical?

We offer a full range of pre-employment health screening.

What we can’t see.

If you have chest pain, severe breathing difficulty or a serious head injury these should be managed at your local A and E department.
